Kati Basti (Lower Back Oil Therapy)
Kati Basti is a local Ayurvedic therapy for the lower back. A ring of dough made from black gram is placed on the lower back, and warm medicated oil is poured inside and kept at a steady warmth for a set time. It is used for lower back pain and stiffness, often as part of a joint-care programme. (Despite the name, it is not an enema; “Basti” here means a container holding the oil.)

- A course of several sessions is usual.
What the Evidence Shows
In a 2024 randomised trial of chronic low back pain, adding Kati Basti to yoga gave similar improvements in pain and disability to yoga alone, although only the combined group showed improvement in physical health scores [1]. A randomised trial of poultice therapy in low back pain found better results when treatment was matched to the stage of the condition [2], and a small study reported benefit from a Basti programme in lumbar spondylosis [3].
A systematic review noted that adverse events are rarely reported in studies of Ayush treatments for spinal problems and may be under-reported [4]. Overall, Kati Basti appears safe in suitable patients, but its specific added benefit is not yet established.
